Optimae LifeServices is seeking a full-time Service Coordinator. The Service Coordinator leads the service delivery and service planning process to ensure the provision of services based on customer desires and goals.

Qualifications:
- Be at least 18 years of age
- Licensed driver with automobile insurance
- Background check required which includes Child and Dependent Adult Abuse and driving record check
- Degree preferred but not required
Duties:
- Ability to work with individuals experiencing mental illness and developmental disabilities.
- Ability to communicate with customers, their families, and provider agencies.
- Some weekends and holidays may be required.
- Coordinate and lead the service delivery and service planning process to ensure the services are based on customer desires and goals.
- Coordinate and lead staff coaching, using pro-active problem-solving, team based methods for customer-driven services
- Provide on-call support by carrying a cell phone/pager.
