EDUCATION & WORK EXPERIENCE
- Must be at least 18 years of age.
- A high school diploma or GED is required.
- A bachelor’s degree from an accredited college or university is preferred.
- With a bachelor’s degree from an accredited college or university, a minimum of one (1) year of experience in the delivery, planning, coordination or administration of human services is required.
- With an associate degree in a human services field, a minimum of two (2) years of experience in the delivery, planning, coordination or administration of human services is required.

JOB SUMMARY
The position of Residential Coordinator was created to provide capable team leaders with more autonomy and responsibility. This lessens the daily responsibilities of the Service Coordinator, which will allow for the Residential Coordinator to oversee more sites. The duties of the Residential Coordinator are based on three areas: coordination of the site, having a supervisory role, and coordination of customer care.
DUTIES AND RESPONSIBILITIES
Coordination of Site
- Ensure the site is clean and equipped with all needed supplies (i.e., cleaning products, furniture, kitchen utensils, paper products). Report concerns at the Monday morning business meetings or as needed.
- Make sure the staff have all needed material to do their job effectively (i.e., contact notes, staff log, time sheets, incident reports, leave requests).
- Responsible for dispersing team member weekly site schedule and finding replacements for shifts that need to be covered. Report any over time to Associate Program Director. Turn weekly site schedules into your Service Coordinator.
- Make sure that the customers have all their medications and all other medical forms (i.e., blood sugar flow sheet, schedule II count off sheet, blood pressure flow sheet). Check each morning to ensure that all site team members are signing off when monitoring medication administration. Turn monthly forms into your Service Coordinator.
- Turn in staff’s completed time sheets daily.
Supervisory Role
- Schedule biweekly team meetings.
- Schedule monthly or bimonthly coaching sessions with all team members. Turn these forms into your Service Coordinator.
- Conduct team member evaluations. Turn into your Service Coordinator.
- Participate in disciplinary actions and exit interviews with Service Coordinator.
- Be available to answer all team member questions.
- Train new team members and conduct individual specific training and complete on boarding forms. Turn on boarding forms into your Service Coordinator.
Coordination of Customer Care
- Provide at least 24 hours of direct customer services weekly.
- Attend all doctor appointments with the customer and turn in doctor visit form to the Service Coordinator. If you must schedule a doctor’s appointment then this needs to be communicated to the Service Coordinator.
- Make sure that the customer is scheduled for recommended routine blood draws and utilize blood draw forms.
- Keep quarterly chart requirements updated and/or completed (rights restrictions, progress notes).
- Keep medication information sheet and physician visit forms updated.
- Attend all TCM and ICP meetings.
Participation in On-Call
- Residential Coordinators will carry the On-Call phone on a 12 week rotation, starting Monday at 5 p.m. to Friday at 8 a.m.
- A Service Coordinator will be the backup for the Residential Coordinator if needed.
- The Residential Coordinator will take over the On-Call on Friday at 8 a.m. and remain the On-Call until Monday at 8a.m.
- The Residential Coordinators will be responsible for finding coverage during their week of On-Call including the weekend when the Service Coordinators are the primary contact.
The Residential Coordinator will physically work at the site if they cannot find appropriate coverage for a shift.
PHYSICAL EXPECTATIONS
- Ability to lift and carry:
- up to 25 lbs. occasionally.
- up to 40 lbs. rarely.
- Ability to climb stairs while carrying objects.
- Normal vision with correction.
- Normal range of hearing with corrections.
- Frequently perform daily duties which involve standing, bending, pivoting, squatting, kneeling, and crouching.
