JOB DESCRIPTION SUMMARY
Responsible for direction of all home health clinical services within a designated geographic service region. Establishes, implements and evaluates goals for providing home health services that support the mission statement, core values and philosophy of Optimae Home Health Services.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- Manages and directs management team to ensure safe, effective, and appropriate home care services. Meets with management team on a regular basis. Meets with management team on a regular basis.
- Is available at all times during regular business hours and as needed to provide support and assure quality care delivery to home care customers.
- Plans, directs, coordinates and monitors the delivery of all direct and indirect services to Optimae Home Health Services home care customers.
- Participates in Optimae Home Health Services strategic planning activities.
- Stays current on available community resources, health care costs, and industry trends through self-education and access to outside educational opportunities.
- Assists in the planning and development of Optimae Home Health Services structure, clinical and administrative policies and procedures. Participates, develops and administers Optimae Home Health Services budget in the designated region.
- Receives and reviews monthly team members’ productivity information with management team.
- Assists in the development of organizational policy and procedures and assures compliance.
- Assists with periodic review of policies and updates. Communicates changes to the management team.
- Establishes and implements systems to inform customers and their families of the scope and nature of home health care services and community resources.
- Attends case conferences and other clinical meetings to facilitate coordination of care. Maintains liaison relationship with attending physicians.
- Identifies admission criteria for new customers and evaluates eligibility for home health care benefits through third-party payers. Provides customers and families with information about covered services as well as those services requiring private pay.
- Conducts weekly and monthly review of records to ensure continuing home health care customer’s eligibility for the covered services. Monitors open and closed charts regularly, and participates in the quarterly clinical record reviews.
- Implements and assists with the evaluation of the orientation program for new personnel.
- Provides directions and resources to coordinators and supervisors to assist in continuous improvement of services provided by Optimae Home Health Services Staff.
- Assists supervisors in managing clinical teams and planning. Facilitates problem-solving sessions to enable management team to resolve customer and/or reimbursement source issues.
- Provides routine management on-call backup for all clinical staff.
- Evaluates the performance of clinical supervisors and assists them in developing skills and techniques to evaluate performance of staff that report to them.
- Plans and implements in-service and continuing education programs to meet specific and ongoing training needs of Optimae Home Health Services personnel. Establishes and implements routine OASIS training and education and other agency specific education.
- Establishes educational programs and information regarding appropriate documentation practices.
- Monitors employee turnover, overtime, and absenteeism. Takes appropriate actions to address problems/issues.
- Assures clinical records are maintained in compliance with state and federal laws including Medicare, Medicaid, state standards, and conditions.
- Oversees data collection for quality management, statistics and reports for evaluating Optimae Home Health Services performance.
- Participates in hiring, evaluating, and terminating Optimae Home Health Services personnel.
- Maintain adequate and appropriate inventory supplies and equipment for providing services to Optimae Home Health Services customers.
- Establishes and implements priorities for services within the limitations of available resources.
- Evaluates Optimae Home Health Services customer base and focuses services toward diagnosis-specific client care needs, when indicated.
- Provides leadership to management team and support staff in identifying Optimae Home Health Services/customer needs and opportunities for quality improvement. Identifies systems to recognize customer needs and to measure the outcomes of Optimae Home Health Services interventions. Utilizes this data to provide direction for Optimae Home Health Services improvements.
- Serves on Optimae Home Health Services Inc. Core Team.
- Establishes Quality Improvement teams to address priorities for improvement activities. Identifies data collection requirements and reports to evaluate the effectiveness of programs to ensure quality of customer care.
- Performs other related duties and responsibilities as deemed necessary.
POSITION QUALIFICATIONS
- Graduate of an accredited nursing program with a current license as a Registered Nurse in the state of Iowa.
- Baccalaureate degree in nursing or related field, or equivalent experience. Master’s Degree preferred.
- OASIS certification preferred.
- Minimum of two (2) years management experience in Home Health or related health care organization.
- Demonstrated ability to supervise and direct clinical personnel.
- Knowledge of business management, Medicare, Medicaid, and state regulations asapplicable.
- CPR certified.
- Excellent written and verbal communication skills and strong interpersonal skills including the ability to market and relate tactfully with customers and other members of the community.
- Must be licensed driver with automobile insured in accordance with the state and/or Optimae Home Health Services requirements.
