JOB DESCRIPTION SUMMARY
Manages, supervises, coordinates, evaluates, and develops customer care teams to ensure quality care delivery and appropriate case management within the Optimae Home Health Services scope of services and policies; state, federal, and local laws; and the Nurse Practice Act. At Optimae Home Health Services the Clinical Supervisor fulfills the role of Clinical Manager as required by CMS. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- Manages and directs the team of field staff, ensuring safe, effective, and appropriate home care services.
- Available at all times during regular business hours and as needed to provide support and assure quality care delivery to home care customers.
- Receives referrals. Determines services required and the ability of Optimae Home Health Services to meet needs. Assigns appropriate clinicians to cases.
- Meets with Case Managers and field staff on a regular basis to provide guidance and information related to specific issues. Provides direction to teams to ensure that customer needs are met and services are provided according to the care plan. Assists clinical staff in establishing priorities, setting goals, and evaluating progress toward goals.
- Conducts internal case conferences with clinical staff to ensure safe, effective, and appropriate home care services.
- Collaborates with the Optimae Home Health Services Regional Director in establishing operating budget needs and priorities for the department.
- Receives monthly team members’ productivity information from the Regional Director and reviews with clinical staff.
- Coordinates 24-hour on-call coverage by Registered Nurse. Evaluates quality of on-call services.
- Provides routine on-call backup management for all clinical staff.
- Develops working relationships with other health care professionals in the community and families to identify resources available and to ensure access of information to customers.
- Provides support and direction to Optimae Home Health Services staff, health care customers, and families related to appropriate and available health care resources.
- Educates Optimae Home Health Services staff on clinical services, policies, and procedures as needed.
- Facilitates problem-solving sessions to enable Case Managers and other staff to resolve customer and/or reimbursement source issues.
- Stays current on available community resources, health care costs, and industry trends through self-education and access to outside educational opportunities.
- Ensures accuracy, completeness, and timelines of clinical documentation in accordance with Optimae Home Health Services policies and procedures, regulatory requirements, and industry standards.
- Provides agency established educational programs and information regarding appropriate documentation practices.
- Provides management to team and support staff in identifying Optimae Home Health Services customer needs and opportunities for quality improvement.
- Assists with marketing, public relations, and discharge planning by participating in departmental meetings as needed.
- Assists quality improvement teams with data collection for the Quality Improvement Plan established by Optimae Home Health Services.
- Reviews OASIS data collection to ensure accuracy and consistency with requirements. Communicates with clinicians and provides on-going OASIS support as indicated to achieve compliance with collection and reporting timelines.
- Identifies and implements changes in clinical and/or operational practice based on the findings of the Quality Improvement Plan.
- Interprets and enforces human resources policies and procedures in a fair and consistent manner.
- Assists in the screening and interviewing of Optimae Home Health Services personnel and makes recommendations to the Regional Director. Assists in the orientation of personnel.
- Conducts timely performance evaluations in accordance with the Optimae Home Health Services policy.
- Provides on-site supervision per Optimae Home Health Services policy and as needed to determine staff competency and respond to educational and developmental needs.
- Follows Optimae Home Health Services guidelines for disciplinary actions. Documents all disciplinary actions in accordance with Human Resource policies and legal guidelines.
- Assists in monitoring employee turnover, overtime, and absenteeism. Takes appropriate action to address problems and issues.
- Collaborates with the Optimae Home Health Services Regional Director and other clinical management staff to ensure proper staffing of qualified and competent personnel.
- Promotes personal safety environments for customers and co-workers. Demonstrates knowledge of safety and infection control practices by compliance with policies and procedures and regulatory requirements. Assesses safety of environment and takes the initiative to help prevent accidents and promote safety. Recognizes and responds appropriately to potentially unsafe situations.
- Performs other duties as assigned.
POSITION QUALIFICATIONS
- Graduate of an accredited school of professional nursing. BSN certification preferred.
- Current license as a Registered Nurse in the state of Iowa.
- Minimum of two (2) years’ nursing experience in home health care or equivalent experience with one year of supervisory experience.
- Complies with accepted professional standards and practices.
- Excellent verbal and written communication skills and strong interpersonal skills.
- Knowledge of reimbursement sources and documentation requirements within home health care.
- Must be a licensed driver in the state of Iowa with automobile insured in accordance with Optimae Home Health Services and state requirements.
- Current CPR certification.
