EDUCATION & WORK EXPERIENCE
- A high school diploma or GED is required.
- A bachelor’s degree from an accredited college or university is preferred.
- Experience preferred in the field of human services.
JOB SUMMARY
The Intake Coordinator will be responsible for overseeing all incoming referrals for site services and all other programs as needed (community integration, community services, etc.). Will oversee initial meet and greet for customers the referral team would like to assess beyond initial review of identifying information. Coordination of the onboarding of new customers into the site program or other programs as needed.
DUTIES AND RESPONSIBILITIES

- Communicate with customers, co-workers, staff supervised, the public, and other providers of service.
- Maintain close communication with referral sources including IHH workers, Case Managers, local hospitals, and other outside sources of referrals.
- Attend Residential Options on a biweekly basis each month. Maintain an organized system for record keeping on all incoming referrals including, but not limited to, application for services, Notice of Decisions from Optimae LifeServices regarding referral status, and all identifying information for the customer utilized for referral review process (social history, medications, contact information, current provider information, etc.).
- Attend agency referral meetings at least once a week. During the referral meeting, present current site openings, new referrals and updates on current referral via referral meeting notes.
- Coordinate with appropriate Service Coordinators to attend this meeting in collaboration with intake coordinator.
- Obtain needed releases of information to gather additional information when necessary.
- Take thorough notes of meeting to present at the next agency referral meeting.
- Coordinate with the designated Service Coordinator a move in date for new customers.
- Coordinate any assistance that may be needed with room preparation or physical move of customer.
- Coordinate the transfer of medications from one pharmacy to another if needed. Assist in obtaining written prescriptions to deliver to the specific pharmacy.
- Compile customer specific initial intake paperwork which can be obtained from the Optimae LifeServices intranet.
- Complete Customer Specific Training requirements with the Residential Coordinator, Service Coordinators, and on-call participants during clinical rounds within two weeks of the customer entering services.
- Complete the initial customer chart documents including the initial contact note, crisis plan, safety plan, emergency support numbers, releases of information, medication sheet, rental lease agreement, and face sheet.
- Communicate with the billing department, HR department, records department, and the finance manager the planned effective date of services for new customers, the site location, their funding source and specific level of services, and who is the identified payee.
PHYSICAL EXPECTATIONS
- Ability to lift and carry:
- up to 25 lbs. frequently.
- up to 40 lbs. occasionally.
- over 40 lbs. rarely.
- Ability to climb stairs while carrying objects.
- Normal vision with correction.
- Normal range of hearing with correction.
- Frequently perform daily duties which involve standing, bending, pivoting, squatting, kneeling, and crouching.
