EDUCATION & WORK EXPERIENCE
- Must be at least 18 years of age.
- A high school diploma or GED is required; bachelor’s degree from accredited college or university preferred.
- LPN or Medication Management Certification required.
- Medication Management course completed within first six months of hire.
- Experience in medication management preferred.
JOB SUMMARY
The Medical Coordinator leads in assisting the health needs of customers. These duties include, but are not limited to, the scheduling of appointments and updating medication changes, and informing staff of such appointment dates and changes to medication. The Medical Coordinator will be part of the staff meetings and give updates on any health and medication issues that need to be addressed. These duties help with the coordination of the site as well as the coordination of customer care. This position will require some on-call shifts and weekend availabilities. The Medical Coordinator will also serve as a Site Team Leader and will report to the Program Director.
DUTIES AND RESPONSIBILITIES
Coordination of Site
- Ensure customers have all their medications and all other medical forms.
- Do quarterly and annual reviews of medication forms.
- If controlled medications are on site, ensure rules and procedures are set in place and all staff are trained on these procedures.
- Update files as needed.
- Sit in on new referrals to discuss medical issues that will need to be addressed when a customer moves into a site.
- Make Home Health referrals as needed. Communicate with Home Health weekly as needed.
- Check and restock First Aid Kits at sites.
- Ensure the site is clean and equipped with all needed supplies and disinfectants. (i.e., masks, gloves, Clorox wipes, thermometers).
- Make sure all PRN medications in the sites are not expired. Dispose and replace as needed.
- Type up and disperse all incident reports Monday through Friday within 24 hours.
Coordination of Customer Care
- Communicate all medical changes to the SC (service coordinator), RC (residential coordinator), and other house lead staff.
- Assist with making all initial appointments and follow up appointments. This includes assisting in medical, psychiatric, and specialty care appointments as needed. Communicate these appointments with the RC and the house lead staff.
- Communicate with house lead staff regarding any doctor’s orders from appointments and ensure this information is being communicated to the DSPs. Ensure all house staff are aware and trained on all medications for all customers in each site.
- Help set up and check in via online patient portals for any applicable medical facilities or offices.
- Ensure all medications are secured at the sites and the documentation is in place for each customer.
- Review each new month’s medications with the house lead staff and the RCs to ensure all medications and MARS forms are correct and correspond according to the physician’s prescriptions. Check and change MARS forms when medication changes occur throughout the month. Ensure staff is signing MARS forms twice per month.
- Ensure customers who get prescribed injections are receiving them according to doctor’s orders.
- Talk with the pharmacy and coordinate with them for medications as needed.
- Gather unpassed medications and destroy them or return them to the pharmacy. Return empty cassettes to pharmacy at the end of / beginning of a new month.
- Dispose of any sharps and expired medications appropriately as needed. Ensure sites have sharps containers.
- Ensure all medical records are coming to the office and put in the customer’s files, including Optimae doctor appointment forms and discharge forms after appointments.
- Observe and check on those customers with diabetes and other health concerns that need consistent monitoring. Educate customers on healthy eating habits, meal planning and medication management.
- Keep medication information sheet and physician visit forms updated.
- Full-time medical coordinators will provide 20-25 hours a week of billable hours. Part-time medical coordinators will provide 8-10 hours per week of billable hours. This enables the medical coordinator to build a rapport with the customers regarding their routines and needs.
